The engine computer saw the voltage from the upstream oxygen sensor on bank 2 (the cylinder bank that does NOT contain cylinder #1) stay too low. This sensor measures oxygen in the exhaust so the computer can balance the fuel mixture. A low reading usually means the sensor reports a constantly lean (too much air) condition or has lost its signal.
